ClubShop Online Shopping Missing Order Form
PLEASE READ - We are unable to post any transactions until the store or vendor provides us with the purchase verification. Most purchases can take up to 10 business days to be reported to us after the merchandise has been shipped or the service or travel has been used and in some cases (Hotels, Plane Tickets, Cruises, Car Rentals, etc.) orders can take much longer to be verified and added to your Transaction Report. Submitting the missing order form does not guarantee that your order will be credited to your Transaction Report, but we will do our best to research your order and respond back to you. Although most merchants complete their investigations in a timely manner, it is possible for some investigations to take 30 days or longer. If our research and inquiry provides us with a verified order, then we will credit your Transaction Report for your missing order and notify you that it has been credited. We will also notify you if we are unable to confirm an order with the store or vendor. Any missing orders that a store or vendor is unable to confirm will not credited to your Transaction Report. |

| 2. You must completely fill out each section below before you submit this form. |
| 3. Forms submitted with incomplete information and/or without a copy of the original invoice/receipt will not be investigated. You must include the dated email headers of your original email invoice/receipt. |
| 4. If you do not use the proper procedure and form, your order will not be investigated. |
