The term "Member" as used in this policy refers to all levels of Membership in ClubShop Rewards.
Members may place their ClubShop Rewards business in a company name at the time of registration if the following criteria are met: - The company is a legally registered business in the state and/or country in which you live.
- You have a FEIN (Federal Employer Identification Number) number (USA) or valid social security number of Member or similar requirement (outside the USA.)
- You are the legally registered owner or officer of this company.
* If you place your Membership in a company name, your commissions will be paid to that company (they will not be paid to an individual's name).
If the company name meets all the above requirements, you may place your ClubShop Rewards membership in your company name at the time of registration.
If you wish to place your business in a company name after you have registered as an individual we must receive the following information from you:- A notarized letter from the Member stating they wish the Membership to be placed in a company name. List the name of the company, address, etc. This letter must also state that upon receipt and processing by ClubShop Rewards of this request, the Member will immediately forfeit their Membership in ClubShop Rewards and the resulting or potential downline from this position as well as any ClubCash or income derived, or to be derived from it. SAMPLE LETTER
- A Copy of the registered DBA for the company, if applicable.
- Also if:
- The company is a corporation, submit a copy of the Articles of Incorporation.
- If the company is a partnership, submit a copy of the partnership agreement.
- If the company is a sole proprietorship, the legal registration for the company.
- The company is a corporation (C, S, LLC, LLS) , submit a copy of the Articles of Incorporation.
- Copy of the FEIN document issued for company or valid social security number of the owner (or similar requirement outside the USA.)
- A notarized letter from the company on company letterhead requesting that the membership be placed in the company name. This letter must also contain the contact individual for this Membership (include full name, address, phone number, email address). This letter must be:
- Signed by the legally registered owner or officer(s) of this company and must state that the owner(s)/officer agrees to the ClubShop Rewards Terms of Agreement.
- State the method of payment the company will use for payment of subscription fees (Debit/Credit card, Money Order, Paypal, Moneybookers, Check). SAMPLE LETTER.
Upon on receipt of this completed paperwork, ClubShop Rewards will place the membership in the company name.
To remove a company name from your ClubShop Rewards Membership
To remove a company name from your ClubShop Rewards Membership we must receive the following:
- A notarized letter on company letterhead from an officer or owner of the company stating they wish to be removed from the Membership and the Membership placed in an individuals name. This letter must also state their acknowledgment that upon receipt and processing by ClubShop Rewards of this request, they will immediately forfeit their membership and the resulting or potential down-line from this position and any rewards points or income derived or to be derived from it. - A notarized letter from the individual who will assume the Membership that they have read and agreed to ClubShop Rewards Terms of Agreement and that they accept full responsibility for the Membership (list ID Number) and the payment of Membership fees. SAMPLE LETTER
Upon on receipt of this completed paperwork, ClubShop Rewards will remove the company name from the Membership and place it in the individual's name.
