For the purpose of this policy, the term "Member" refers to all levels of Membership in ClubShop Rewards. This policy went into effect on 9/1/2000 and was revised on 10/1/03.

If both spouses wish to become Members in ClubShop Rewards, they may be sponsored jointly in an individual Membership or they may have individual memberships.

If a Member desires to add their spouse to their Membership, they may do so by following the instructions below:

The present owner of the Membership must send a notarized letter to ClubShop Rewards HQ stating s/he wishes to add his/her spouse to the Membership. The letter must state the Member's ID number and the following information:

1. Member's full name and social security number (USA only)
2. Spouse's full name and social security number (USA only)
3. A statement by the Member stating s/he wishes to add their spouse to their ClubShop Rewards Membership.
4. A statement by the Spouse indicating s/he agrees to the ClubShop Rewards's Terms of Agreement (if Partner) and has agreed to assume a membership with his/her spouse.
